Enter your "test" information and choose "Mail In Payment" as the payment choice. Choose "Next" and then "Process Payment" - this is where I'm getting the white screen... no relevant error logs, either... please help!

For some reasons, I could not see the issue on my local development server. Could you please submit a support ticket sending me administrator and FTP account of your site so that I can check and solve it for you ?
Bad news : Your site seems to be hacked. The file phpmailer.php (a Joomla core file used for sending emails) was modified. I had to re-upload that file and everything works well now. See
