Memebership pro

Hi there,
I have a little trouble with Memebership Pro: I think I've set all access, but the "user profile" menu item gives an almost completely blank image, only something is visible in vertical bar on the left side. (see attached file)
Please help me what is wrong?

Must be something special on your site causing this issue. Please submit a support ticket sending us:

1. Super user account of your site

2. Link to the page

3. An account which can be used to access to that page and see the error

I'm going to check to see why the issue happens and get it sorted
