Display Category Selection

How can I add a (multi) Category Selection area to my subscription form?

I don't understand this question. But maybe you can create a custom field (Type Select) to allow users to choose categories in the subscription form?

Try to go to Membership Pro -> Custom Fields, create a custom field, then check to see whether that's what you need?
