Unpredictably but fairly often people on events get multiple registration emails.
Im using Mandrill to send emails and I was wondering what might be triggereing the same people to be emailed. Is the trigger happening before the system has a chance to update the database?

multiple registration emails mean the email users received when they register for the event ? If so, I don't think it could happen
However, if you are talking about the reminder email, then Yes, it can happen. We have solution available but need to setup cron job. Will try to document it soon

It is the original registration email being duplicated. It maybe a mix between this extension and mandrill - has anyone suggested this as an issue before?

I don't see anyone has this issue so far. Maybe you can try to setup Events Booking on a test site which don't have Mandrill and check it to see whether you see the error ?

Is it possible if a few people were booking almost at the same time and they submitted their booking within a few seconds of each other before the database had time to update that they could all trigger emails for all?

It is not possible. Each registration is independent with each other, so I don't think it could happen
As I said, you should test with a fresh installation of Joomla without Mandrill installed, and you should see which one causing the issue
