Attaching form to article

For the life of me cannot figure out how to attach my form to an article.
I have read the tutorials and followed word for word and still nothing.
I have made certain that the the plug-in was published, the form was marked published and tried all combinations of the {pmform 1} {pm form FORM_ID} ....
I have also made sure that there were no errors while installing. I have reinstalled this plug-in even a few times to make sure I didn't miss anything.
One my MAC i did not get any errors but I tried to reinstall on a windows pc and I did get one error one time that said Jinstaller:Install: Cannot find XML setup file.

I am just lost here.
Can you please advise?
