Unfortunately, PMF doesn't support multiple-pages form feature, so it cannot work like in link you sent me. In PMF, all the fields must be in the same form page (one page only).

If PMF can support just only one page, I think I can make the different price banner on first page and link to PMF form which inside can set multi choice for price.
So I have more question, if there is new user which has never register to website before. How PMF will manage that user? Is it possible for user to register via PMF?
