Custom Payment Amount Form was created by abarrow
I've searched docs and the forum and I can't find it. In the Demo, a "Custom Payment Amount Form" is shown. 

How do I create one?

Hello

You can create a Form yourself. And assign the amount field to that form. Then when users access to the form, they can enter custom payment amount into the Amount field and they will be charged for that amount
