Form variables not decoding, optional items appear

I am using Payment Form on my website menofscars.com/jrh to accept payments for retreats.
(The payment form is under the menu "Retreats" and the last item is called "Retreats."


I have set a title for the Payment called "Retreats" and have made the "address 2" to be shown but optional.
However, on the pament form, I have no title llisted for the payment, and the "addresss 2" has a red star next to it which (i assume) indicates required.

Any ideas?

Hi Mark

It seems you forgot to choose the form in the menu parameter. Please look at the attached image for more detail . Just choose a form in the dropdown and everything will work fine .
