[FORM_TITLE] not generating in email subject line

I have done some offline testing and discovered a potential issue.

When I mark an offline payment as paid the confirmation email that goes to the customer has a subject line without the proper form name.

Thanks for reporting. We actually didn't support for tags in subject of that email. I just edited code to have it works

Please get this file, unzip it, upload to components/com_pmform/Helper folder, then check it again and update us with the result
