Questions about Payment Form extension

Filled-in fields are blank was created by Alan Henness
I have been testing the form and noticed that three fields are blank even though I had entered text into them. They are blank everywhere: in Payment Manager: Edit Payment, the confirmation email, the notification email and in the csv export.
Two of the fields are required but one is not. See screenshot of one of the fields and the complete list of fields.
Have I missed something? Everything else seems to be working properly.

That's strange to me. Could you please help submitting a support ticket sending us super admin account of the site, link to the form and tell us what's wrong so that we can check it?

I need to publish the form this weekend so, as a workaround, I've created new custom fields to replace the core fields that weren't working. This seems to be working OK.
It is odd if no one else has come across this, particularly since it concerns core fields.
