Questions about Payment Form extension

I cannot get this payment form to calculate price's correctly and need assistance. I read the instructions but had no luck.
Question 1.
This is at the top of my form?
Please enter information on this form to make payment for . The amount you need to pay is $0.00 .
I did not place it there and cannot make it go away and when I select my price choices, nothing changes.
Please see screenshot for remaining questions.

Here are my answer to your questions :
=> You can remove this message by changing configuration from back-end of the extension. Go to Payment Form->Configuration, look at the Form Message config option and remove the message .
Regarding your questions about custom fields :
- Everything you setup is correct. Just one thing need to be changed, that is you need to set Fee Field to Yesinstead of No.
- The fee formular in the current version only is used for Textbox custom fee field. As explained in the tutorial, You can use [FIELD_VALUE], [FORM_AMOUNT] and math operator : +, -, * , / to canculate fee value for this field .
Hope my explanation is clear to you. Please let me know if you have other questions .

Thanks for your reply. My forms calculation is still not working.
I set the form field to "Yes" and I tried several variations of the multiplier but nothing has worked.
I tried this
[FIELD_VALUE]+
and this
[FIELD_VALUE]+[FORM_AMOUNT]
and this
+,[FIELD_VALUE],[FORM_AMOUNT]
and this
[FIELD_VALUE],[FORM_AMOUNT]+
So which is it?

Hi
As I said, the fee formular only used for textbox custom fee field. It doesn't work with the dropdown custom field you are using .
For your need, you just simply set the Fee Field property for this custom field to Yes and everything will work properly. If you need more explanation, please contact me via Skype, YM, MSN or Gtalk and I will be happy to explain it to you .

Hi
I think It is very simple :
1. When you edit the field, you will see an option called Fee Field -> Just set it to Yes.
2. Leave the fee formular textbox empty .
And It will work properly . My gtalk nick name is extension4joomla@gmail.com. Please talk to me .
