[requests] for PM form to make it better was created by thepiston

1) add "show on confirmation page" option for each field. Some things like messages do not need to show. Default it to Yes/checked.

2) Make "Enable Captcha", "Confirmation Step", "Show term and condition checkbox", and "Term and condition article" show for each Form and not just in global Configuration since a form may need different settings than global
