Problem with field names

It seems that I can't get the content of the fields through to the email... I've tried changing everything a hundred times.

Here are three screen shot, you can see the information doesn't show in the email.

What am I doing wrong?

That looks strange to me. I don't have any clue about why it happens. Could you please submit a support ticket sending us super admin account of your site and the link to the form. We are going to check to see what's wrong and get it sorted for you
