calculating formula problem

Could you assist me with getting the formula right for calculating the final payment on this form:

I need to have (tutition * quantity) + supply + other_amount entered into the Total Amount to pay

On a related note, I have the quantity field set to show up when someone selects something from the Tuition field. But even though I have it set to be just under the Tutition field in the ordering it is still showing up at the bottom of the list. Am I doing something wrong there?
