How to set up my form?

I bought the Payment form extension because I am building a Family Reunion website and I want my family to be able to pay for their registration and t-shirts online. I am not understanding how to set up the form in an article. I have tried, but I keep getting an error message about a table set to null!

Okay, great, but I have it set up somewhat now. My only issue is getting it to calculate a total amount when more than one item is selected. I users to be able to purchase a registration type and add t-shirts. I have no idea of how this will work. I have the form in a test article for now, but need to get it up and ready so that family can begin registration.

I figured out how to find the URL to my form from this forum. It has good information, great! The link to my form is www.henderson-chargois.com/index.php

I need for family to be able to choose a Registration type such as Single Adults and/or Children 3-17 (first 2 children) and/or Children 3-17 (additional children) and put in the quantity. Then I would like for them to be able to put in what t-shirts they want (quantity and size) and all these amounts calculated to give them a total amount. I do not want the Total amount to show at the top of the screen, but at the bottom where they will pay for their complete registration.

I know that you said you need my super user name and password. Where can I send it to you confidentially?
I do not want to post on this public site.

Ooops forgot to tell you the name of the form. I have so much that I keep trying for trial and error, that I want to be sure that you can get to the correct form. The name of the form that I created is called Test Registration Form and it is in the category What's Happening Now.
