Look on the forms tab and you will see a list of forms which you have created with a column called "id" all the way to the right. These are the correct responses to the 'form_id'. In other words {pmform 1} should work, unless you created and then deleted that form. Just pick the numbers off the form tab. By the way, where did you find the command in the documentation? I only recently found the command itself on this forum.
