Next Button Doesn't Go Anywhere

I have setup Payment Form on my website but when I click the "Next" button, it doesn't go anywhere:

There might be some javascript conflict on your site causing the issue. Please submit a support ticket sending us admin and FTP account of your site.

It seems to be going to a different page, not a confirmation page:
