Donation Amount Showing On All Pages

The donation amount and Donate button are showing up on all the donation pages.

If you go here: www.prayerrevival.tv/content/prayerrevival-partner and scroll to the bottom, click the amount and click donate, when you get to the next page to fill in the information, just scroll down to the bottom of the page and you will see the information repeating itself.

How to I stop it from showing on all the other pages?

I received complain from Paypal that you used someone's Paypal account to purchase the extension and I will have to refund money for this transaction .

That's something with css of your website. However, since you didn't pay for the extension, I won't check it for you .

I don't know where you got your information from, but it has been paid for, credit card charged and I expect some resolution on this issue.
