Form Message and Title

1. I created a form and where it says "Form Message" it's blank and that's how I want it as I'm inserting this form in an article. However, beside that box it says "Available Tags :[FORM_TILE], [AMOUNT]. Leave it empty to use the message in configuration area.", so the message in my configuration is showing instead. What do I need to do so I do NOT have any message showing in the form that I created?

2. Also, because my form is inserted in an article I would like to have the option to remove the Form Title completed. How do I do this?
