Multiple payment options / form opening messege

hello..

i have installed the Payment form..i have three payment prices..they appear fine..but at the top of the form is a messege that has an amout of 0.00...how i get rid of it

also under membership type this is a blank check box that is checked when you come to the page

1=> Go to Payment Form -> Configuration, look at Messages tab, you will see a message and can change it to remove the 0.00

2=> Regarding the blank Checkbox, you will need to edit the custom field, make sure at the end of "Values" attribute of that custom fields, there are no blank lines. (please remove all the blank lines). After that, it will work as expected.
