Multiple payment amounts on one form?

It looked like this was possible, but I'm not sure how to configure it. I tried to add a custom field and used something like this:

Values

Fox Cities
Green Bay
Madison
Metro Milwaukee
Southeast
Member at Large

Then I selected Yes for Fee field and entered:
200.00
250.00
300.00
375.00
250.00
200.00

I thought I would see a list of values and their fees. But it doesn't seem to apply the fee to the value?
If this isn't what the Fee field is for, what is it for and how does it work?

Thanks. I think this is a very good start. Just needs some tweaking. There is some left over donation language in the forms etc. but a really good start for a component that is really needed.
