Last Name Field not in export

Last Name Field not in export was created by eradio
I am using Payment Form version 1.4 and the last name field does not appear in my CSV export. How can I make this appear? It is a required field.

Hi

Please send me administrator and FTP account of your site so that I can do a quick check. Version 1.4 is a very old version, and to be honest, I don't remember exactly how to configure it at the moment.
