I believe the reason is that you forgot to choose the form when you create menu item. Could you please edit the menu item, look at the menu parameter (at the right) and choose the form from the list of form you created ? After that, the issue should be solved.

I just checked and found out the issue. It seems the issue causes by SH404. I had to disable Sh404 for Payment Form component and It works well now.

Also, I corrected one issue with your settings. You installed Payment Form plugin in the Payment Plugins section and it is not correct. I had to uninstalled it and installed via Joomla plugins management.

Everything works well now. Please go to your site to check it and let me know the result !
