Creating dependent dropdown fields? was created by azurelink

How do I create dependent fields in Payment Form?
I'd like to display two different dropdown fee fields, depending upon whether a checkbox is selected, or not.
Is it possible to set this up?

Unfortunately, the extension doesn't support dependent custom fields in the current version ! Will try to implement it in future release !
