Use of Custom Fields on Booking form

The manual shows how to use additional custom fields in services as well as booking pages.  However, I cannot find how to assign a custom field to a table in the database.

I can create a custom field on the booking page, and enter the data, but it is not included in the email confirmation nor in the orders table.
I looked in the orders tables and do not see the custom field listed

UPDATE: .................  I was able to find the data in the table(s) app_sch_fields and app_sch_field_data.
                                  The data does now appear in the confirmation page. But they do not appear in the sent email

I currently have the site on my laptop using XAMPP.... I'll put on another PC, assign FQDN and get it to you. Will take me a day or so.
