- is it possible to get an appointment without email confirmation? (to avoid customer spam problems)
- is it possible to avoid the cart? (free appointments)

-> Yes, OS Services Booking has feature send reminder emails and sms

-> Yes

- Is it easy to modify and extend the days and hours of a service, when for example you need to open on a holiday? (flexible time slots, not fixed)
-> OS Services Booking is appointment booking system. It only allows customers to make booking on timeslots in a day. It doesn't allow to have through-days timeslots. Please note this important point.

- is it possible to get an appointment without email confirmation? (to avoid customer spam problems)
-> Yes

- is it possible to avoid the cart? (free appointments)
-> Yes, but even you enable cart option, you still can disable payment. Thus, customers won't need to make payment to complete the booking.

Many thanks Dam, concerning the flexible time-slots, the scenario is something like that: suppose the customer has a timeslot on monday from 09 am to 11 am, but the next monday (and only the next monday, not all mondays) he wants to open till 12 am. Is he able to modify this single day without changing next days?

You can use feature "Custom working time" to setup the different working time on specific date(s)
