Provide pdf attachment for customer

Hi,
we need the customers to download and bring in a personal information pdf form in relation to a specific service. What is the best way to supply this form (e.g. custom field, attachment in email template, link to form on our site)?

Hi Ian,
Currently, OS Services Booking supports "File" custom field, but that file can only be shown/used on Checkout form
