Hi there,
It's possible to add custom fields in this type:

Additional option is in price $1.00(p). And client type what number of this option want (x). Finally is p*x=total price for this option

Hi Wojciech,
Currently, OS Services Booking doesn't have feature to add Custom fields like you want. If you want to have it, we can developer that feature for you with additional cost. I will wait for your response.
