"Tell a Friend" form doesn't submit

Click on Submit and nothing happens. The form does validate - if I leave a field blank I get a popup message. I have StopForumSpam disabled in configuration, and have configured Joomla for PHP Mail and Sendmail - both working properly. I do have Breezingforms installed and forms with it are working fine.
