CSV Import: Not Working (Can't Select State/City) was created by swiftmedia

I am having trouble with the CSV Import feature.

I get to Step 3, but I can't Select the State or City AND it requires all information be filled-in.

Please help.

I don't see the attachment photo. But can you please send me your site information with super account to: damdt@joomservices.com. I will log into your site and check the problem for you.
