I want to integrate a payment system for PagueloFacil which is the Panamanian version of PayPal. (Paypal is not useable here)
What would be involved in doing this, can I adapt an existing gateway?

This is hard to answer question. You need to read their integration document and try to develop it, maybe base on existing os_paypal payment plugin
