horrible looking subscribe form - help wanted

I know this is a CSS question in the end, but I ran out of options. My subscribe form looks horrible. Does anyone know where to start looking to change things so it looks like this example:

Mine is here:

As you can see, it's all in one column, and horrible.

Go to Membership Pro -> Configuration, set Twitter Bootstrap Version config option to Twitter Bootstrap Version 2, then check it again. The error should be gone
