Input fields layout in form

Using Helix3 template with Bootstrap3. Some Input fields are not showing ok (input-group-addon and checkbox).
In config I set Load Twitter Bootstrap Css In The Frontend to No and Twitter Bootstrap Version to version 3.

For this, we might have to add some custom css to solve it. Could you please submit a support ticket sending us:

- Super admin account of the site
- Link to the page which we can see the error

We are going to check to see why the issue happens and get it sorted
