Membership Pro - Subscription form

Goodday,
We would like to inquire about modifying the default layout for the subscription form and subscription confirmation form.

Currently, the form renders as follows:


However, we would prefer that the form renders something similar as follows:


If this can be achieved simply by modifying CSS can you provide us exactly which files need to be modified and which elements within the CSS need modification?

If this can not be done by modifying the CSS and would require modification of your components we would be open to discuss available options. In this case, I would prefer to have a contact that I can communicate to directly. My work email address is available through my profile.
