Blank page on renewal

Hello there
After upgrading users are unable to renew. After click on the renew button just the header, with no template, is shown:

Subscription Renewal
Please enter information on the form below to process renew subscription for Acadêmico / Governo for 4 meses.

Nothing else is presented to to user. What is going on?

Hello Marcelo

There might be something special on your site causing the error. Please submit a support ticket sending us:

1. Super admin account

2. A subscriber account which we can use to renew subscription and see the error

3. Link to the renewal page

We are going to check to see what's wrong and get it sorted
