Setting up payment using Moneris and having issues

I'm trying to set up credit card payment through Moneris. I changed moneris_helper.php to point to the Moneris test environment and am using the test store and API token. I changed my plan prices to one of the required amounts. There are a few issues:

For renewing and new subscriptions I get the error: "The transaction was not sent to the host because of a duplicate order id". I can see this happening. I'm doing a bunch of testing and (in the code) the order ID is set to row->id (os_moneris.php, line 109). The order ID must be unique. In the Moneris sample code they use a prefix + timestamp.

There is no prompt for a credit card. Maybe I missed a checkbox.

I'm in Canada and the ProcCountryCode should be set to "CA" (part of HttpsPostRequest object).

While testing the TestMode should be TRUE (part of HttpsPostRequest object).

1. Super admin account of your site

2. Link to subscription form

We will check to see what's wrong and get it sorted. Honestly, we haven't touched that plugin for sometime
