default value in checkbox custom field not registering

default value in checkbox custom field not registering was created by Jonathan Lehmann
I have added a opt out field, with a yes/no selection, and set the default to no, but when i go to look at a profile no value is selected.

any ideas why this might be happening?

to clarify i set the default value to "no" and when i view on the profile, neither value is selected
