first name, last name, and uploaded image not displaying in profile or member li was created by Jonathan Lehmann
even though they are turned on in the custom fields, for both member list and profile field settings, none of these items are displaying

That's strange. Could you please edit the custom field, set Show on members list parameter to Yes, then check it again?
It should be displayed on Members List

just checked it. front name and last name are both set to SHow on members list and profile, and is showing in neither

even weirder, middle name, which is set up the same way, displays on both

the only difference between first name, last name and middle name, is that first name and last name are core fields.
