Custom Fee Fields

Hi , is there any kind of document where i can find some example of how use the custom fee formula ?
i am trying to put a custom field with checkboxes for 6 item and for each item the subscriber have to indicate the quantity but i think that i am missing something..
exemple :

check ITEM 1 for quantity X
check ITEM 2 for quantity Y
etcetc

If you still don't understand how to setup after reading the documentation, you can submit a support ticket:

1. Sending us super admin account of your site

2. Sending us link to subscription form

3. Explain how fee should be calculated
