Hello,
I would like to have fields within sentences such as what you see here (noted by red arrows): prntscr.com/a9j44u . the data entered would just need to show on the backend database in a way that is organized such as within the sentences noted in my screen shot. Can you let me know how that can be done? Thank you very much in advance.

I am sorry but this is not something Membership Pro is designed for, so you could not use Membership Pro for this requirement
I am not sure I understand your requirement correctly, but for a requirement like this, I think a form extension (like RS Form) will be better
