new user registration form doesn't appear

Hi,

I had made Membership Pro live on our website and when people went to click on a subscription plan to sign up, they don't see the new user registration form. They only see the fields "username", "password" and "retype password". I am not sure what is wrong since when I was first testing Membership Pro options, I did this live for a while and the form appeared on the page.

My god. Seems someone unpublished/deleted all custom fields for some reasons. Please submit a support ticket sending us super admin account of your site so that we can check and correct the settings for you (hope the fields are still there/not being deleted yet)
