Unwanted addition from deleted module

A little help please - very satisfied otherwise
This is showing up as below in bold italic. Not sure where it is coming from but it was in a module that was trashed and deleted. it comes up after clicking the "Sign up" button for all plans and all categories.

Nope just checked. Not fixed.
it comes up after clicking the "Sign up" button
www.nhcla.com membership - become a member - "choose a plan" - click "sign up"
It comes up just under "Please enter information on the form below to process subscription for "xxxxx"
