Update : My support staff checked and found out that the reason of this issue is because Nico config the extension to not load jQuery library (this library is needed, so you can only set this config option to No if your template has jQuery library loaded already)

HI Tuan, I still have problem with SignUp Page + Blank Page.

I have disabled captcha in Joomla global configuration page and in OS Membership configuration page. I have enabled "Load jQuery Javascript library" but these actions don't save the issue.

Inspector Message:
Failed to load resource: the server responded with a status of 500 (Internal Server Error)

I have open also a ticket about this.
