Display subscription form

when I try to sign on the subscription plan page does not compare the subscription form... it's come out an error

what I should do to set the subscription form to be displayed?

when I click on the sign-in button nothin happens. The browser works untill comes out a message for time out.

One question:
the subscription plans page is only for registered user (already registered), should they registered in membership plans page again ?
If not, how should I configure to subscribe directly the plan choosen?
