Add the word "Other" next to blank amount field?

How can I get the word "other" next to the $ and the blank amount field?

I thought it used to be there but I recently upgraded and noticed it isn't there.

Other questions:
1) How can I unbold the amount descriptions but bold the amounts
2) How can I remove the brackets around the amount descriptions
