The drop-down menu of United States states is empty

Hello,

I recently installed the latest version of Joom Donation, v5.9.11 .  I set the United States as the default country.  On the user front end, the list of states is empty.  There is only the "Select State" option, even in the page source code.

Also, there is an empty text-edit box at the bottom of the donation form page.  It is below the "Process donation" button, and has no text or clue what it is from.  Any ideas about this?

Can anyone help solve this?  Thank you.

How can I create a list of countries and states like the websites below? Is such a thing possible? Where can I get a list of countries and states?
