on my donation form, you can select an amount at first, but after you select a campaign, or change campaign, you an no longer select a pre-defined amount. You can put a number in the "other" box, but it remains impossible to select an amount no matter which campaign is selected.
This is using the "Simple" layout, and doesn't seem to matter if the campaign has its own predefined amount (which do change) or just using global amounts. Also tried various bootstrap integrations.
