Donation Module/Form View

Hello,

I'm looking to create a module that looks like one of the frontend designs your website advertises, but cannot find it. This is the view I am looking at:  joomdonation.com/images/jdonation/jdonation/slide7.jpg

Would anyone know how to create a module similar to this? I like the look of this design better than the donation campaign module.

Hi Chris,
It's not module of Joom Donation, it's the Donation page of specific Campaign. The Blue part at the top is the Campaign information, below is donation form. Here is an example: joomdonationdemo.com/donation/donate/donate-single-campaign
