Custom field with date

Hello there again,

Because our donors have to specify their birth dates (for tax reasons), I tried to set up a custom field where you can enter a date. I created a new custom field with name and title, I chose "date" as field type and "past date" as Datatype validation. However, in the frontend, all I get is one empty text field (see attached image).

I realized that I only have this problem when I am using the simple layout. With default layout, I get at least the Title (label), but there is no calendar field where to choose the birth date from, only a blank text field.

Any idea how to solve this issue? thx, Christoph

Thanks for the fast answer. Unfortunately, I am not allowed to give access to our admin section, so may I ask, if you have any idea what the problem might me and I am gonna dig for a solution on my own?
