Joom Donation has Custom fields management tool which you can use to add/edit/delete fields that will be shown in Donation forms.

Hi,
Please turn on configure option "Custom fields based on campaign", and then, when you add custom fields, you will able to assign those fields to specific campaign. And those fields will be shown only in Donation form at the Donation form of that campaign.
